SPECIAL CONTRIBUTION OFFER DECEMBER 1 TO FEBRUARY 11, 2024 GUIDELINES:

Minimum contribution of $250 and Maximum contribution of $500 qualifies for matching 1:1
Designation of contribution must be for Annual Fund- SHARE (this is the fund that 50% comes back to District in 3 years)
All contributions must be mailed directly to The Rotary Foundation (14280 Collections Center Drive, Chicago, IL 60693-0142)
Contributions must be made between December 1, 2023 to February 11, 2024
If mailing contributions as a group within your Club, use The Rotary Foundation's Multiple Donor Form.
If mailing contribution as an individual donor, use The Rotary Foundation Contribution Form.
To obtain the matching, send copy of the above form to Janice Kane, via email at msjkane1@gmail.com

Contributions made online:

Many of you make your contribution online just by logging in to your Rotary account and using the DONATE button. After you contribute you will receive an email from the Rotary Foundation validating your contribution.

Please send a copy of that receipt to Janice Kane so we can transfer your points.

The rules are simple: If have a Recognition Amount less than $8,500 you are eligible to participate. Simply email me at smilesrotary5790@gmail.com and let me know how much you are contributing to the Polio Plus fund– the amount must be between $250 and $500.

Then, either donate directly to the Rotary Foundation by going online at www.rotary.org (please be sure to log into My Rotary before donating) or mailing your contribution

As soon as your contribution posts, I will release your match.
